Office Policies

All initial intake paperwork must be completed and submitted 24 hours prior to appointment date and time.
Incomplete paperwork, or failure to submit paperwork will result in an appointment cancellation.

Payment for session is due 24hrs prior to date and time of appointment.

 Failure to submit payments 24 hours prior to date and time of appointment will result in cancellation.

*Booking of any appointment requires a stored credit card on file*

We kindly ask that all clients are logged into the telehealth platform, at least 5 minutes prior to the beginning of the session.

We ask that clients are located in an area, with adequate lightening and minimal noise/distraction.

Patients will be granted a 15 minute grace period.  After 15 minutes appointment will be CANCELLED. A late fee of $50 will be charged to account. 

All appointments must be rescheduled within 24 hours prior to date and time of appointment.
Failure to reschedule appointment prior to 24 hours will result in a cancellation fee for $50.

A no-show will result in a $50 fee per occurrence. After 2 occurrences client will be dismissed from practice and ineligible to book any future appointments with provider.    

A mandatory two week follow up is required for all newly written prescriptions. This required follow up is to be sure that medication is properly tolerated without any adverse reactions.

Please be advised that upon initial prescribing no more than 30 days of a prescription will be given. After first initial follow-up, additional refills will be supplied. 

At no point in time will Unashamed Minds, LLC, prescribe ANY controlled substance, this includes any benzodiazepines or psycho stimulants.
